This application is for first through fifth graders only. Sixth graders will need a teen (middle and high school) application completed. Please click here for that application. 

 

There are two parts to the after-school program application -- Part A and Part B. You must complete BOTH parts for the full application to be considered.

PLEASE NOTE: The final page of Part B cannot be completed online. At your earliest convenience, please come into the Club at 163 Martin Luther King Jr. Drive in Winder to receive a hard copy of this page to complete the application process. We apologize for any inconvenience, and we thank you for your cooperation. 

 

The Annual Membership Fee of $50 is nonrefundable.
